Wholesaler/Reseller Account Activation - Step 1

1. I would like to setup a wholesaler/reseller account but do not currently have a website.

If you run a club, please upload scanned proof of your club. Please note: your name must be illustrated on the proof submitted. (Examples: Shop utility bill, hall receipts, official letter from association, magazine article). If you have a website you do not need to complete this section, instead you can go straight to the Application Form.

Files accepted in .jpg format only. Please try not to use spaces in the filenames. Use an underscore 'my_filename.jpg' or hyphen 'my-filename.jpg' instead.

2. I would like to setup a wholesaler/reseller account and already have a website.
